The family would like to thank Dr. Hurwitz and Krrynne and the staff at Northern Nephrology. We would also like to thank Dr. Cleveland and Dr. Pabst and the staff at each office. The staff at the HK Freedman Renal Center became family to us. They treated Jack with kindness and respect and he felt very safe there.

The same was true at Meadowbrook Healthcare in the Subacute Rehab Unit, on 2W/Oakwoods and with his 'taxi" drivers Amy and Michael taking him to dialysis and appointments.

Jack will always be remembered by those who knew and loved him as a delightful, kind and gentle man. He was a wonderful husband and father and an even greater Grandfather. He will be greatly missed.
